We are commissioned to support Integrated Care System’s (ICS’s) across our geography with CVD Prevention through three key stages of; identification of patients with potential CVD, initial treatment to address their condition and optimisation of treatment to aid management of their condition. The Health Innovation Network CVD programme will focus on Heart failure as a national programme in 2025/26. Locally within the West of England, as a response to system need, we are also focusing on:
- Lipid management to support high cholesterol
- Inherited high cholesterol known as Familial Hypercholesterolemia
- High blood pressure (Hypertension)
- Reducing CVD related health inequalities
Naturally, in supporting our systems with their CVD prevention work we offer a wide range of innovation services. For example, we can provide horizon scanning to search current and developing innovations that will enhance patient care. Plus, we have an evaluation and insights service which can support demonstrating the impact of new ways of working.
